This program is designed to assist people with

• Getting consumers of nursing homes or institutional settings
• Preventing consumers from going into nursing homes or institutional settings
• Assisting with HRA 2010e 

If you are not already a Medicaid beneficiary, enrollment assistance is available. This program enhances our other services for transitioning and remaining out of institutions. We partner with other organizations to provide much-needed services, including:

1. Finding an accessible home in the community
2. Making the current home accessible
3. Applying for subsidized rent, if eligible
4. A Coordinator assigned to help consumers survive and thrive in the community,
5. Applying for and Keeping Disability Benefits
6. Assistance with healthcare insurance appeals for more home care, medical equipment, and supplies
